If you have signed up for an appointment to be vaccinated at Animas Surgical Hospital, please take a moment to review the Pfizer COVID19 Vaccine Consent Form. To speed up the registration process, please print, sign, and bring this form to your appointment. Please note that you do not need insurance, you do not need to be a current patient of Animas Surgical Hospital, and you will not be responsible for any out-of-pocket costs.
