Insurance / Payment / Billing

Contracts with insurance providers are subject to change. Please call our office to verify if we participate with your provider and plan.

Your insurance plan may require a co-payment, co-insurance, and/or a deductible. For services that require a co-pay (physician office visit, urgent care visit, occupational medicine visit, or emergency room care), you will need to pay at the time of your visit.

For surgical services and inpatient hospital care, you will receive a bill for your co-insurance, deductible, and any other non-covered services. For your convenience, we accept checks, Visa, MasterCard, Discover, and American Express.

You can also pay your bill online.

If you do not have health insurance, you can apply for help with your bill by calling (970) 385-2373 or make an appointment in person with our Benefits Coordinator by calling (970) 403-1330 to see if you qualify for financial assistance.
